An Apple a Day focuses on health promotion. As a way of giving back to the community, we have done a series of wellbeing talks, most recently focussing on the effect of food on mental wellbeing in a lecture called 'Food for thought' in collaboration with the Bright Minds Academy and in GP surgeries as part of the 'Eat, Move, Sleep, repeat' campaign together with an NHS Paediatrician and dietician.
We use the best evidence base but understand that life isn't a one size fits all. Wellbeing is what works in the context of your life. Making realistic changes at the pace people can cope with is why this works so well.
An invaluable pocket guide containing 10 everyday questions for care home workers to ask themselves each time they care for a resident together with a training programme has been developed on behalf of NW London Collaboration of CCGs.
The success of this training, which has now trained 40 trainers and delivered training to all 8 CCGs in north west London along with further uptake in Barnet and Camden CCGs, lies in its co-design with the carers and nurses from Rowanweald and Buchanan Court Care Homes and a steering group of members from across NW London in health and social care.
Between 2016 and 2018 this Health Education England sponsored, double HSJ Value award winning projected developed a new model for health social care to work collaboratively to improve care for care home residents in the last phase of life. This project created:
- a 5 day last phase of life multi-professional training programme in collaboration with St Luke's Hospice.
- implemented the first pilot of the Sutton Vanguard Homes of Care 'Red Bag hospital transfer scheme' and
- championed the use of Coordinate My Care, digital urgent care plan platform in Harrow which lead to a 196% increase in usage in the borough within the project life span.
We use actors to train real life scenarios with a multi-professional faculty. Using real life cases multiple themes can be taught simultaneously and brought to life within the attendees own environment such as the care homes (pictured above) or in a simulation centre using actors.

This award winning multi-professional education programme sponsored by Health Education England, sought to improve learning by integrating attendees from across different disciplines to improve care for the dying.
A core curriculum was developed alongside bespoke training following a needs assessment. Tailor making training is key to ensuring training is meaningful to attendees.
The Challenges in End of Life Care programme and Harrow PACT Project 5 days programme curriculums have been successfully condensed into one day study days for GPs, allied health care professionals and care home staff by collaborating with experts in their field.
Study days have included:
- Advance care planning (bespoke programmes for health and social care)
- Advance care planning for people with Learning Disabilities
-Challenges in end of life care (more technical training including prescribing and legal and ethical frameworks).
